PITYRIASIS ROSEA<\/strong>
\nPityriasis rosea (P.R.) เป็นโรคผิวหนังในกลุ่ม papulosquamous disease ซึ่งพบได้ 1-2% หายเองได้ มักพบในวัยรุ่นและผู้ใหญ่อายุ 15-40 ปี ทั้งเพศหญิงและเพศชายพบได้เท่ากัน ผื่นมักจะขึ้นตามลำตัว ต้นแขนและขา อาการคันไม่มากนัก
\nลักษณะทางคลินิก
\nลักษณะผื่นอันแรกที่เรียกว่า herald patch มีรูปร่างกลมรี สีแดง ขอบชัด มี scale บางๆ ที่ขอบของผื่นเรียกว่า collarette scale ซึ่งจะมีขนาดใหญ่เส้นผ่าศูนย์กลางประมาณ 2-5 ซม. อาจเกิดบริเวณใดของร่างกายก็ได้ ประมาณ 1-2 สัปดาห์ต่อมา จึงมีผื่นลักษณะเดียวกันแต่ขนาดเล็กกว่า เส้นผ่าศูนย์กลางประมาณ 0.5-1.5 ซม. ค่อยๆ ทยอยขึ้นตามมา มีอาการคันเล็กน้อย แกนตามยาวของแต่ละผื่นจะขนานกับ line of cleavage ผื่นกระจายอยู่ตามลำตัว ต้นแขน ต้นขา ดูรวมๆ ตามตัวมีลักษณะคล้ายต้น Christmas ซึ่งเห็นชัดเมื่อดูด้านหลัง  ในเด็กอาจจะพบผื่นที่ใบหน้าและเท้าได้<\/p>\n

พยาธิกำเนิด
\nสาเหตุของโรคนี้ยังไม่ทราบแน่ชัด สันนิษฐานว่า อาจจะเกิดจากเชื้อไวรัส เนื่องจากโรคนี้เป็นระยะสั้น และหายเองได้ แต่จนถึงปัจจุบันยังไม่สามารถตรวจพบเชื้อ และทางพยาธิวิทยาก็ไม่พบการเปลี่ยนแปลงที่จำเพาะ
\nการวินิจฉัย
\nโรคนี้วินิจฉัยได้จากประวัติและลักษณะทางคลินิก ไม่มีการตรวจทางห้องปฏิบัติการที่จำเพาะ
\nการวินิจฉัยแยกโรค
\n1. Herald patch ต้องแยกออกจาก
\n– Tinea corporis โดยการทำ KOH preparation
\n2. Secondary syphilis ผู้ป่วยทุกรายควรเจาะเลือดตรวจ VDRL
\n3. Drug eruption
\n4. Guttate psoriasis
\n5. Viral exanthem
\nการรักษา
\nโรคนี้หายได้เอง จึงให้การรักษาตามอาการ ควรหลีกเลี่ยงสิ่งที่อาจระคายเคืองผิวหนัง
\n1. ให้ทายา emollient และรับประทาน Antihisamine จะช่วยลดอาการคัน
\n2. ทา Steroid cream จะทำให้หายเร็วขึ้นและลดอาการคัน
\n3. Phototherapy ใช้รักษาในรายที่เป็นมากและเรื้อรัง จะช่วยให้ผื่นหายเร็วขึ้น<\/p>\n

โรคนี้หายได้เองภายใน 3-6 สัปดาห์ แต่บางรายอาจจะเรื้อรังและหายใน 3-4 เดือน<\/p>\n","protected":false},"excerpt":{"rendered":"
