เกลื้อน , pityriasis versicolor<\/strong><\/h1>\n

โรคเกลื้อน (Pityriasis versicolor)
\n เกิดจากเชื้อ Malassezia furfur (Pityrosporum orbiculare, P. ovale) ซึ่งเป็น lipophilic yeast และพบเป็น normal flora ในรูขุมขน โดยปกติเชื้อจะอยู่ในรูปของ yeast form การจะเกิดโรคเกลื้อนขึ้นหรือไม่จึงขึ้นกับ precipitating factor ที่สำคัญคือ ผิวหนังมัน, มีเหงื่อออกมาก, มีภาวะภูมิคุ้มกันต่ำ, อยู่ในที่อากาศร้อนชื่นเป็นต้น ภาวะเหล่านี้จะกระตุ้นทำให้เชื้อเจริญเติบโตเปลี่ยนเป็น hypha form ซึ่งเป็น pathogen<\/p>\n

ลักษณะผื่นผิวหนังเป็นผื่นราบสีจางกว่าผิวหนังปกติมีขอบเขตชัดเจนและมีขุยบาง ๆ(ในบางรายผื่นอาจมีสีแดงหรือสีเทาคล้ายมีขี้เถ้าได้) ผื่นจะเริ่มเกิดรอบ ๆ รูขุมขนก่อน แล้วจึงขยายมารวมกันเป็นปื้นใหญ่ คนไข้มักไม่มีอาการ ผื่นมักเป็นบริเวณที่มีต่อมไขมันหนาแน่น เช่น บริเวณหน้าอก, หลัง, ต้นแขน ในเด็กอาจพบที่หน้าได้
\nการวินิจฉัย จากลักษณะทาง clinic
\n ส่องดูด้วย Wood ‘s lamp ผื่นจะเรืองแสงเป็นสี Golden yellow
\n Lab :- ขูดขุยมาย้อมด้วยน้ำยา KOH จะพบshort septate hyphae และ round หรือ oval yeasts
\n :- Scotch tape technic : ใช้ Scotch tape แปะที่ผื่นเพื่อให้ติดเอา scale ออกมา แล้วหยดดูด้วยสี methylene blue จะพบลักษณะตัวเชื้อเช่นเดียวกับการทำ KOH ติดสีฟ้าทำให้เห็นชัดขึ้น
\nการวินิจฉันแยกโรค
\n1. Pityriasis alba มักเป็นวงขาวขอบเขตไม่ชัดเจน ชอบเป็นบริเวณใบหน้าหรือบริเวณที่โดนแดด
\n2. Vitiligo ผื่นขอบเขตชัดเจน ด่างขาวเหมือนกระดาษไม่มีขุย
\nการรักษา
\n1. Topical agent : ควรแนะนำให้คนไข้ทายาที่บริเวณตัว, แขนและขา (ถึงแค่หัวเข่า) ให้ทั่วถึง ถึงแม้จะเป็นผื่นอยู่เพียงแค่หย่อมเดียว
\n– 20% Sod.thiosulfate ทาวันละ 2 ครั้งหลังอาบน้ำเช้า-เย็น เป็นเวลา 2 สัปดาห์
\n– 50% Propylene glycol ทาวันละ 2 ครั้งหลังอาบน้ำเช้า-เย็น เป็นเวลา 2 สัปดาห์
\n– Zinc pyrithione shampoo ใช้ทาผื่นทิ้งไว้ 15 นาทีแล้วล้างออกทุกวันนาน 2 สัปดาห์
\n– 2.5 % Seleniumsulfide shampoo (Selsun) ใช้ทาผื่นทิ้งไว้ 15 นาทีแล้วล้างออกทุกวันนาน 2 สัปดาห์
\n– Topical imidazole cream ทาวันละ 2 ครั้งนาน 2 สัปดาห์ ไม่นิยมใช้เพราะราคาแพง เนื่องจากต้องทาเป็นบริเวณกว้าง<\/p>\n

2. Systemic agent ใช้ในรายที่มี extensive lesion, ใช้ topical treatment ไม่ได้ผลหรือมี relapse
\nยาที่นิยมใช้มากที่สุดคือ Ketoconazole เนื่องจากยาถูก excrete ได้ดีทาง sweat, มี MIC ต่อเชื้อต่ำกว่า และราคาถูกกว่า Azole ตัวอื่น
\nวิธีการใช้ยามีหลายแบบ
\n Single dose regimen: ขนาดที่ให้แตกต่างกันตั้งแต่ 400 mg- 800 mg ครั้งเดียว
\n Daily dose regimen: ขนาด 200 mg\/d นาน 5-10 วัน
\n ไม่ว่าจะรักษาด้วยวิธีใด จะต้องแนะนำคนไข้เสมอว่าหลังรักษาถ้าได้ผล ขุยบาง ๆ จะหายไป แต่ผื่นที่มีสีจางกว่าผิวหนังปกตินั้นจะยัง persist อยู่ต่อไปได้อีกประมาณ 3-4 เดือน
\n3. Prophylactic treatment ใช้ในรายที่มีประวัติเป็นซ้ำบ่อย ๆ อาจให้
\n– Ketoconazole 200 mg\/วัน ติดต่อกันนานเป็นเวลา 3 วัน ทุกเดือน
\n– Ketoconazole 400 mg. Single dose เดือนละครั้ง
\n– 1% selenium sulfide shampoo ฟอกตัวเดือนละครั้ง<\/p>\n","protected":false},"excerpt":{"rendered":"
