เชื้อราแคนดิดาที่ผิวหนัง,ช่องคลอดและปาก ,oral ,vaginal and superficial candidiasis
\nSuperficial Candida infection <\/strong>
\n เชื้อที่ทำให้เกิดโรคได้บ่อยคือ Candida albicans ซึ่งพบเป็น normal flora อยู่แล้วในบริเวณเยื่อบุปาก, ทางเดินอาหารและช่องคลอด โดยอยู่ในรูปของ yeast form เมื่อมีปัจจัยส่งเสริมที่เหมาะสม จึงจะทำให้เกิดโรคขึ้น โดยตัวเชื้อจะเปลี่ยน form เกิดมีทั้ง yeast, hyphae และ pseudohyphae
\n Superficial candidiasis มีได้หลายแบบ แต่จะขอกล่าวถึงเฉพาะ Oral และ Cutaneous candidiasis เท่านั้น<\/p>\n

Oral candidiasis
\nแบ่งเป็น 5 clinical form
\n1. Acute pseudomembranous candidiasis (oral thrush) เป็นชนิดที่พบบ่อยที่สุด เกิดจากมี invasion ของ candida บริเวณ buccal mucosa ลักษณะที่พบบ่อยคือ เป็นฝ้าขาว ๆ เปื่อยยุ่ยคล้ายคราบน้ำนมที่บริเวณกระพุ้งแก้ม, เยื่อบุเพดานปาก ถ้าเขี่ยเอาฝ้าขาว ๆ นี้ออกจะพบว่า mucosa ข้างใต้มีการอักเสบ เป็นปื้นแดง พบบ่อยมากในเด็กเล็ก ๆ และในคนสูงอายุ ในรายที่เป็นรุนแรงอาจลามไปที่ลิ้นและ pharynx ซึ่งเป็นลักษณะที่พบบ่อยในคนไข้ HIV positive
\n2. Acute atrophic candidiasis (antibiotic candidiasis) อาจเกิดได้เองหรือเกิดหลังจาก pseudomembrane ของ thrush หลุดออกไป มักพบในคนไข้ที่ได้ antibiotic หลาย ๆ ตัวหรือคนไข้ที่ได้ topical, inhaled หรือ systemic corticosteroid พบบ่อยบริเวณด้านบนของลิ้น
\n3. Chronic atrophic candidiasis เป็นชนิดที่พบบ่อยที่สุดในคนไข้ใส่ฟันปลอม มักมาด้วย chronic erythema และ edema ของ palate ที่อยู่ใต้แผ่นฟันปลอม คนไข้มักไม่มี
\n4. Candida cheilosis or perleche มาด้วยผื่นแดง, เปื่อย, แตกเป็นร่อง (erythema, maceration, fissuring) และมีอาการเจ็บมากที่มุมปากทั้งสองข้าง พบบ่อยในคนที่ชอบเลียปากหรือในคนแก่ที่ไม่มีฟันนหรือฟันขบกันไม่ดีทำให้มีน้ำลายค้างที่มุมปากบ่อย ๆ
\n5. Chronic hyperplastic candidiasis (candida leukoplakia) เป็นฝ้าขาวที่หนาติดแน่นพบในคนไข้ chronic mucocutaneous candidiasis
\nการรักษา Oral candidiasis
\n1. แก้ไขปัจจัยที่ส่งเสริมให้เกิดโรค
\n2. Antifungal treatment
\n2.1. Local: Nystatin oral suspension 400,000-600,000 unit ให้อมกลั้วคอแล้วกลืนวันละ 4 ครั้งนาน 7-10 วัน
\n2.2. Systemic : ใช้ในรายที่เป็น chronic form หรือมี esophagitis ร่วมด้วย
\nKetoconazole 200-400 mg.\/day นาน 3-5 สัปดาห์
\nหรือ Fluconazole 50 mg.\/day นาน 7-10 วัน<\/p>\n

Candida infection ของผิวหนังและ Appendages
\n1. Intertrigo
\nเป็นการติดเชื้อ Candida ที่บริเวณซอกพับต่าง ๆ ของร่างกาย มักเกิดจาก Candida albicans พบบ่อยในคนอ้วน ๆ ในคนไข้ที่เป็นเบาหวาน หรือเกิดซ้ำเติมบนผื่นผิวหนังอื่น ๆ เช่น flexural psoriasis, diaper dermatitis, Darrier’s disease เป็นต้น
\n พบบ่อยบริเวณขอหนีบและใต้ราวนม
\n ลักษณะผื่นจะเป็นปื้นแดงมีขุยบาง ๆ บางครั้งจะแฉะและเปื่อย แต่ที่สำคัญคือจะพบมีผื่นแดงหรือตุ่มหนองขนาดเล็กกระจายอยู่ที่บริเวณขอบ ๆ ของผื่นเดิม (sattellite lesions)
\n การวินิจฉัยโรค: จากลักษณะผื่น และการตรวจหาเชื้อจากกล้องจุลทรรศน์จะพบ budding yeast , pseudohyphae และ septate hyphae
\n การรักษา : topical imidazole cream หรือ topical nystatin cream ทาวันละ 2 ครั้ง ควรให้นานประมาณ 2-3 สัปดาห์
\n ในรายที่รุนแรงให้ systemic treatment ได้แก่
\n ketoconazole 200 mg OC x 10 days หรือ 200 mg BID x 5 days
\nหรือ itraconazole 100 mg\/d x 15 days
\n2. Interdigital Candidiasis
\nอาจพบบริเวณง่ามมือหรือง่ามเท้า เป็นผื่นขาว ๆ แฉะอาจมีร่องแตกตื้น ๆ (Erosio interdigitalis blastomycetica) มักพบบ่อยที่บริเวณซอกเท้าระหว่างนิ้วที่ 4 และ 5 ของเท้า คนไข้มักจะมีประวัติโดนน้ำมากหรือใส่รองเท้าอบชื้น ต้องแยกจากผื่นที่เกิดจากเชื้อกลากและ Cram negative bacteria
\nการรักษา: topical imidazole หรือ topical systatin ทาวันละ 2 ครั้ง นาน 2 สัปดาห์ ถ้าต้องใส่รองเท้าอบประจำ อาจช่วยลดความชื้นได้โดยใช้แป้งโรยหรือทา
\nบริเวณที่เป็นด้วย 2% acetic acid หรือ potassium permanganate 1: 5000 solution
\n3. Candida paronychia
\nInfection ของหมวกเล็บ (nail fold) มักพบในคนไข้ที่ต้องโดนน้ำบ่อย ๆ เช่น แม่บ้าน
\nและพวกที่มี peripheral vascular disease ชอบเป็นที่นิ้วชี้และนิ้วนางได้บ่อยที่สุด
\n Candidal paronychia มัก involve บริเวณ proximal nail fold อาการจะค่อย ๆ เป็น เริ่มจากมีบวมแดงที่หมวกเล็บทำให้ตัวหมวกเล็บไม่ติดกับแผ่นเล็บ (nail plate) อาจมีหนองใต้หมวกเล็บได้เล็กน้อย ถ้าเป็นนาน ๆ จะทำให้เล็บที่งอกออกมาใหม่มีร่องขรุขระ <\/p>\n

การวินิจฉัยแยกโรค : bacterial paronychia มักมี acute onset , involve ที่ lateral nail fold มากกว่า และมีอาการเจ็บปวดมาก
\n การรักษา : topical imidazole ในรูปของ lotion จะดีกว่า cream ให้ทาที่หมวกเล็บจนกว่าอาการบวมจะหายไป ซึ่งอาจต้องใช้เวลานาน 3-4 เดือน และต้องแนะนำคนไข้ให้หลีกเลี่ยงการโดนน้ำ โดยอาจต้องใช้ถุงมือสวมเวลาทำงาน
\n ในรายที่มี nail deformity มาก อาจจำเป็นต้องให้ ketoconazole รับประทาน
\n4. Candida balantitis มักพบในรายที่ภรรยาเป็น vaginal candidiasis ด้วย ลักษณะผื่นเป็นปื้นแดง, เปื่อยแฉะ อาจมีขุยหรือฝ้าขาวบาง ๆ มักเป็นบริเวณ glans penis, prepuce การรักษาต้องหา predisposing factors และรักษาภรรยาไปพร้อมกันด้วย
\nงครอบคลุมทั้งเชื้อ candida, dermatophyte และเชื้อราตัวอื่น ฟอกตัวเดือนละครั้ง<\/p>\n","protected":false},"excerpt":{"rendered":"
