Aimovig ยาฉีดรักษาไมเกรนใหม่ (ant-CGRP) ยาต้านไมเกรนชนิดใหม่ รักษาและป้องกันไมเกรนโดยยับยั้งการกระตุ้นโปรตีนที่เรียกว่า แคลซิโตนิน ยีน รีเลท เป๊บไตด์ ซึ่งทำให้เส้นเลือดขยายตัว
Aimovig (anti-CGRP) is a monoclonal antibody that blocks the activation of a certain protein that can produce a migraine attack. This protein, called calcitonin gene- related peptide (CGRP), can cause blood vessels to dilate and cause inflammation and migraine headache pain.

Aimovig received Food and Drug Administration (FDA) approval in May 2018. It was the first medication to be approved in the CGRP antagonist class of drugs.

CGRP คืออะไร

calcitonin gene- related peptide (CGRP), a vasoactive neuro peptide that can cause blood vessels to dilate and cause inflammation and migraine headache pain.
เป็นโปรตีน (vasoactive neuropeptide) ที่หลั่งเวลาปวดศีรษะ และทำให้เส้นเลือดขยายตัว ก่อให้เกิดการหลั่งสารกระตุ้นการปวดอื่นๆออกมา CGRP เป็น neuropeptide มีกรดอะมิโน 37 ตัว มีฤทธิ์แรงในการขยายหลอดเลือดและเป็นสารก่อการอักเสบ สารนี้หลั่งจาก trigeminal nerve fibers ซึ่งเส้นประสาทนี้ส่งไปยังเยื่อหุ้มสมอง และใบหน้า เชื่อว่า CGRP เกี่ยวข้องกับการกระตุ้นและการส่งสัญญานความเจ็บปวดภายใน trigeminovascular pathways โดยทำงานร่วมกับ neuromediators อื่น เช่น substance P, bradykinin มีการศึกษาที่สนับสนุนว่าหากลดปริมาณ CGRP ได้จะช่วยป้องกันและการรักษาโรคปวดศีรษะไมเกรนได้อย่างมีประสิทธิภาพยิ่งขึ้น

ห้ามใช้ในคนที่แพ้ต่อยา หรือกลุ่ม erenumab-aooe
ไม่แนะนำในหญิงตั้งครรภ์ แพลนจะตั้งครรภ์ ให้นมบุตร

ผลข้างเคียงของ Aimovig

แพ้ ผื่นคัน บวม อาจเกิดได้ในคนที่แพ้ยาตัวนี้
ท้องผูก
ความดันโลหิตสูง

การบริหารยา

เป็นยาฉีด มีขนาด 70 และ 140 มิลลิกรัม

มีการพัฒนายาในรูปแบบต่างๆในช่วงไม่กี่ปีที่ผ่านมา เช่น

ยากลุ่ม gepants ออกฤทธิ์เป็น CGRP antagonist เช่น telcagepant , olcegepant แม้จะมีประสิทธิภาพ แต่ผลข้างเคียงมากจึงยังไม่ออกมาใช้ อย่างไรก็ตามยังมีการวิจัยยากลุ่มนี้ใหม่ๆ ออกมา เช่น atogepant , rimagepant , ubrogepant
monoclonal antibody ที่ขัดขวาง CGRP เช่น eptinezumab , fremanezumab , galcanezumab และล่าสุดที่ออกมาใช้แล้วคือ erenumab (human monoclonal antibody ) และยาที่กำลังอยู่ในการดำเนินการขอใช้ คือ Emgality (galcanezumab)
