โรคบิดไม่มีตัว หรือ Shigellosis เป็นโรคติดเชื้อทางเดินอาหารที่สำคัญซึ่งเกิดจากแบคทีเรียในกลุ่ม *Shigella* แม้ว่าในปัจจุบันเทคโนโลยีทางการแพทย์จะก้าวหน้าไปมาก แต่โรคนี้ยังคงเป็นสาเหตุหลักของการเกิดอาการถ่ายเป็นมูกหรือมูกปนเลือด (Dysentery) ทั่วโลก โดยเฉพาะในพื้นที่ที่มีสุขอนามัยไม่เพียงพอ

Shigellosis is a significant gastrointestinal infection caused by bacteria of the *Shigella* genus. Although medical technology has advanced significantly, it remains a leading cause of dysentery (diarrhea with mucus or blood) worldwide, particularly in areas with inadequate sanitation.

Shigellosis เกิดจากการติดเชื้อแบคทีเรียกลุ่ม *Shigella* ซึ่งสามารถแบ่งออกเป็นหลายสายพันธุ์ เช่น *S. dysenteriae*, *S. flexneri*, *S. boydii*, และ *S. sonnei* เชื้อเหล่านี้มักแพร่กระจายผ่านทาง Fecal-oral route หรือการรับประทานอาหารและน้ำที่มีการปนเปื้อนของอุจจาระผู้ป่วย

Shigellosis is caused by *Shigella* bacteria, which include species such as *S. dysenteriae*, *S. flexneri*, *S. boydii*, and *S. sonnei*. These bacteria are primarily transmitted via the fecal-oral route, through contaminated food or water.

  • Infection Mechanism: Once ingested, the bacteria invade the intestinal mucosa, particularly the large intestine, causing severe inflammation that leads to abdominal cramps and bloody mucus stools.

ระยะฟักตัวของโรคโดยทั่วไปอยู่ที่ 1-7 วัน แต่ที่พบบ่อยที่สุดคือ 24-48 ชั่วโมง

The incubation period typically ranges from 1 to 7 days, with most cases presenting within 24 to 48 hours.

  1. ระยะเริ่มต้น (Early Stage): มีอาการปวดบิดในท้อง (Abdominal cramps) ตามด้วยไข้สูง (High fever) และการถ่ายเหลวเป็นน้ำในช่วงแรก
  2. ระยะรุนแรง (Advanced Stage): อาการถ่ายเหลวจะเปลี่ยนเป็นการถ่ายเป็นมูก (Mucus) หรือมูกปนเลือด (Bloody mucus) โดยผู้ป่วยจะรู้สึกปวดเบ่งที่ทวารหนัก (Tenesmus) อย่างรุนแรง
  3. อาการร่วมอื่นๆ (Associated Symptoms): ปวดศีรษะ, ปวดเมื่อยตามตัว, คลื่นไส้ และอาเจียน
  1. Early Stage: Abdominal cramps followed by high fever and initial watery diarrhea.
  2. Advanced Stage: Diarrhea transitions to passing mucus or blood, accompanied by intense rectal pain or the urge to defecate (tenesmus).
  3. Associated Symptoms: Headache, malaise, nausea, and vomiting.

While it affects all ages, young children and the elderly are at highest risk due to their limited ability to compensate for fluid loss.

นี่คือหัวใจสำคัญของการรักษา หากมีอาการถ่ายเหลว ควรดื่ม ผงน้ำตาลเกลือแร่ (ORS) เพื่อป้องกันภาวะขาดน้ำ

Rehydration is the cornerstone of treatment. Patients should consume Oral Rehydration Salts (ORS) to prevent dehydration.
